Community First Consultation Services will assist the participant in how to select a
representative, as necessary. We will also ensure that the participant or the participant’s
representative understands, agrees with, and can articulate choices within CFSS in ways such as
being knowledgeable in self-directed principles and using person-centered planning principles to
assist the participant to develop their CFSS Service Delivery Plan. Based on self-direction
principles and person centered planning principles, the plan must include:

  1. The participant’s needs and preferences based on their assessed needs;
  2. Desired outcomes within the scope of CFSS;
  3. Natural or informal supports;
  4. Any services and supports to be purchased in CFSS;
  5. Providers of services and goods;
  6. Technical support during implementation of the CFSS Service Delivery Plan;
  7. Options for worker training; and
  8. Any other items required for the CFSS service delivery plan.
